ZDoggMD on Urgent Care Clinics: Access Solution or Fragmentation Machine?

This article discusses ideas from ZDoggMD — Dr. Zubin Damania, MD , Stanford/UCSF-trained internist, founder of the Health 3.0 movement. Full archive: zdoggmd.com | YouTube @zdoggmd

The US now has over 10,000 urgent care centers, up from virtually none in 2000. They serve a genuine function — acute, episodic care for patients who can't wait for their PCP or whose PCP doesn't have same-day appointments. ZDoggMD's view: urgent care is a symptom of primary care failure as much as it is a solution to it.

The Access Argument

When a primary care practice is booked 3 weeks out and a patient has a UTI today, urgent care is the appropriate resource. When the ED wait is 5 hours for a non-emergency and urgent care can see the patient in 20 minutes at a lower cost, urgent care wins on access, efficiency, and cost. These are real benefits that serve real patients.

ZDoggMD acknowledges the access value directly — arguing against urgent care as a concept would be arguing for emergency rooms as the default, which is worse in every dimension. The question is not whether urgent care should exist, but what role it should play in a well-designed healthcare system and whether it's displacing primary care relationship development.

The Fragmentation Problem

A patient who uses urgent care for acute illness, their PCP for chronic disease management, the ED for emergencies, and various specialists for conditions each generates medical records, prescriptions, and care plans that are held in different systems, often inaccessible to each other. Urgent care physicians who don't know the patient's baseline have no way to recognize atypical presentations. Prescriptions from urgent care can interact with medications prescribed by the PCP who doesn't know about the urgent care visit. Care fragmentation is a patient safety issue.

Same-Day Appointments Are Easier When Charts Are Done

Practices that can offer same-day access keep patients out of urgent care and in longitudinal relationships. NoPajamaTime's documentation efficiency creates space to see the same-day patient.
