ZDoggMD on Retail Clinics: CVS, Walgreens, and Amazon — Healthcare Disruption or Relationship Destruction?

This article discusses ideas from ZDoggMD — Dr. Zubin Damania, MD , Stanford/UCSF-trained internist, founder of the Health 3.0 movement. Full archive: zdoggmd.com | YouTube @zdoggmd

CVS Health Minute Clinics, Walgreens Village Medical, Amazon Clinic, Walmart Health — retail giants are moving aggressively into primary care. ZDoggMD has examined this with his characteristic nuance: there is genuine value in what these models offer, and there is a genuine risk they make the underlying problem worse.

What Retail Clinics Are Good At

Retail clinics are genuinely good at episodic, acute care for low-complexity conditions: strep throat, UTIs, pink eye, minor wound care, flu shots, COVID testing. They are convenient, available outside business hours, often cheaper than urgent care or the ED, and don't require an established relationship with a provider. For patients without a primary care physician — a growing proportion of Americans — retail clinics provide access to basic care that would otherwise go to the emergency department.

ZDoggMD acknowledges this access benefit: if retail clinics reduce ED visits for strep throat and UTIs, that's genuinely good for patients and the healthcare system. The access argument is real and shouldn't be dismissed by physicians who are protecting turf more than patients.

What Retail Clinics Can't Do

Longitudinal care. Chronic disease management. The therapeutic power of a relationship with a physician who knows the patient over time. Complex diagnostic reasoning that depends on knowing this patient's baseline. The retail clinic model is optimized for throughput and convenience — which are the exact opposite of what good primary care requires. When retail clinics market themselves as primary care alternatives, they are promising something they can't deliver.

The Relationship is What Retail Can't Replicate

A physician who knows their patient — and has time to give them — is irreplaceable. NoPajamaTime helps you be that physician by eliminating the documentation burden that competes with time.
